Session 9: Its a about a cleaning crew who is fixing up an abandoned mental institution in Massachusetts(I've seen the actual building, its creepy). It s a psychological thriller about how the creepy atmosphere effects the workers. Its pretty good, but I've seen a lot of posters put it on too big of a pedestal.
Last House on the Left: An old Wes Craven film. Its about a couple of scum bag drug addict rapists who go a little to far. The rape scenes are kind of disturbing. Not a huge gore factor.
Edit- when you said quiet I kind of though you meant underrated or no mainstream flicks. But 2 of my 4 choices still fit the bill.
I'd add The Machinist also. Its got a cliche ending, but i thought it was interesting because instead of seeing someone lose their sanity and then get it back in a second like most movies do, the protagonist slowly regains his sanity and certain things become clear as the movie progresses.

Session 9 comes to mind. Very slow paced, but creepy.
Misery

Burnt Offerings comes to mind as the perfect candidate.
It's a little slow, but at the same time, it's creepy as hell, and the premise is basic yet awesome, so it won't put you to sleep.
it's not rare, but it's also not well known. check it out!

Just for the record, I'm not after films that are "disturbing".

Oh, Visiting Hours is a good choice. Just saw it for the first time last weekend and it's AWESOME. Really has some of the best scares ever created. I jumped like 10 times.
Burnt Offerings is good, too. Just hope you can stand slow movies.
The Ring/Ringu are both very creepy and understated
A Tale of Two Sisters plays like a drama for most of it's length and when it gets scary, it gets SCARY!
The Innocents is the best one I can think of. Very eerie and spooky with great acting and locations and a genuinly interesting story.
Tourist Trap- Creepy slasher film with a good cast and creepy villan.
Curtains- An Agatha Christie modernization in a way about 6 actresses who are at a casting call at a director's house and find themselves stalked by one of the creepiest kills ever to grace the screen.

Woot-de-woot-woot on that. It's one of my favourite films and I didn't even think of mentioning it.
The original Phantasm is a slow moving creepy film too.

I know alot of people disagree with me on liking this one, but I'd recommend Silent Night, Bloody Night, a '70's slow paced "slasher" with Mary Woronov.
Also, Let's Scare Jessica To Death. Slow, but worthy.

The Mothman Prophecies is a good one...
Cronenberg has many of them, SPIDER is truly slow, quiet and creepy.
Oh yeah, THE DEVILS BACKBONE and CRONOS by Guillermo Del Toro... damn good...
A TALE OF TWO SISTERS definetly (Asian horror has MANY creepy films)
THE EXORCIST 3 did it for me...
And of course, FRAILTY, THE OTHERS, SIXTH SENSE, THE EYE, STIR OF ECHOES and PI (Yes, Darren Aronofsky's firs outing)
PS: Looking forward to HARD CANDY, THE MACHINIST and DAS EXPERIMENT...

Quiet and creepy is definately what I think of Clownhouse 20 years after its creation, knowing that the director had the child star suck him off backstage.

I think Session 9 is actually quite overrated. I noticed it was mentioned a few times. Its too slow, its a little creepy and the ending was disturbing... but rediculous.
Carnival of Souls is amazing! I cant say enough. Visit Past Horror section for more on that.
And also... Im interested in Burnt Offerings... Id like to hear more about that.
My recommendations, Carnival Of Souls and The Sentinel (from the late 70's, early 80's).

Paperhouse - I know I'll probably get laughed out of here but this creeped me out when I saw it
The Haunting (1963)
Devils Backbone - Guillermo del Toro is a genius
Dead End
Closet Land
For a creepy highy disturbing little flick. I have to recommend the french movie "In My Skin".
I am hardly squemish and rarely disturbed by movies but "in my skin" took the cake. In my skin is about a young women who becomes obsessed with mutilating her body after accidently cutting her leg. That's bascially it, but to watch the metamorphis this girl goes through from picking at a oozing scab to taking chunks of her skin off, to the more extreme almost becomes unbareable to watch.
I have to say this movie might have made me squirm even more than the french rape film "irreversilbe" which is also highly disturbing.
Trust me if you want to feel disturbed and confused and have your jaw hanging on the floor check out "IN MY SKIN"
