This is honestly starting to become a "Whats in the briefcase in Pulp Fiction" topic.

Spoilers
Enough threads on this shit already, jeez. She was psycho lesbien telling her story to the cops at the beginning of the movie, she asks if the tape is recording thus she is telling her side of the story and she isnt going to admit she was the killer, but towards the end of her version it falls apart, thus we have an unexpected ending. It works towards the movies advantage because it never cheated on the viewer infact all you had to do was pay attention the first 5 minutes of the movie and the ending wouldve made sense, well at least more sense then SAW that shit just pushed it, good sequel though. :D

I adore Haute Tension's twist...it's what separates the movie from the rest.

The ending left you with a disturbed feeling and a perfect song, more than can be said of many modern horror films. It was pretty clear cut, just really really disturbing.
