I'd say either:

Cannibal Holocaust

The Ebola Syndrome is supposed to be fucking ridiculous in the gore department. Check out the screens in this review (http://www.mandiapple.com/snowblood/ebolasyndrome.htm). It's a Category III flick, and it's a black comedy. It's only ten bucks on Deep Discount DVD, and mine's on the way. Anyway, in terms of sheer brutality, I'd have to say Cannibal Holocaust. The spike scene gives me chills whenever I see it.
*edit*
Can't forget to mention the Guinea Pig flicks. I've never seen any, but I've heard they're pretty nasty. I want to box set, but I'm not sure if I want to plunk down the money. And I downloaded Shogun's Sadism last night (which I hear is pretty nasty), and I'm going to give it a watch tonight.
*edit 2*
Gah. Forgot to mention Kichiku Dai Enkai. It didn't live up the hype, but it's still pretty nasty. The highlight is when the main girl gets a shotgun blast RIGHT in her cooch.

Wow! I wanna see the Ebola Syndrome. Looks insanely gory and it features Anthony Wong  I love this actor !!
I suggest another asian import Riki-Oh :The Story of Ricky. Really cheesy & funny prison gore fest that's more in the style of Toxic Avenger and Braindead.

The Guinea Pig series are a good suggestion. My recommendations for those (as far as gore is concerned) is probably "The Flower Of Flesh and Blood" or "Mermaid In A Manhole".
"The Flower..." has fantastic dissection scenes that don't cue the camera away for a second, and "Mermaid" has some of the best festering boils I've ever seen.
And as Dirtyfrog mentioned, Riki-Oh :The Story of Ricky is also another great one.
Oh, and did you guys really think Cannibal Holocaust was that gorey? I mean I love the movie...but I don't like when they show you awkward camera angles to hide some of the FX flaws and such (for instance the whole butchering scene at the end...I suppose I just wish they would have had a better close up for that).
I haven't watched it for awhile though. I may just have to give it another view, maybe my opinion will change.

Kichiku: Banquet of the Beasts or Naked Blood. The Asians can really make 'em nice and juicy.
however...
The ultimate 'nasty' in my opinion is Subconscious Cruelty. This movie seems to have only been created to mar the soul with sequence after sequence of 'What the f***!' images. I paid over $100 for my copy and it is extremely rare. I showed it to a friend of mine who is a hardcore gorehound and after viewing it he said he wished he hadn't seen it. It's truly messed up. One sequence involves Christ being raped and killed by demons.
You see it, and you're damned.

yeah. I've got a real flair for the melodramatic. It's serious stuff, though. The August Underground stuff is pretty sick, too. The thing with Subconscious Cruelty, is that it's really hard to imagine what the film's going for. I imagine it's just trying to be the most blasphemous thing ever committed to film. In that, it may have succeeded.

I remember having a discussion about "Subconsious Cruelty" on the "upcoming Horror" section of the boards and the members had quite the debate.
I also remember reading an interview by the director and he came off like the most pretentious man to ever direct film. "The eloquence of the movie is the hatred within man's soul while the evil allows despair... yadda yadda yadda". He talked that way for the whole interview.

From what I have seen it may be Lucio Fulci's beyond. It definitley has the most REAL looking effects. It's just about the only movie I look at where I am convinced the gore could have actually happened. Well thats all for now GOoD JOURNEY my fellow schmoes.

Das Komabrutale Duell is getting a DVD release from Unearthed Films soon. The same guys who put out the Guinea Pigs, Aftermath, and Red Room.
As for Subconcious Cruelty, I liked it. I felt as if I was watching somebody's nightmare that had somehow been captured to film. It really has that dreamlike feel to it.

I think Bijo no harawata (aka Entrails of a Beautiful Woman or Guts of a Virgin 2) is supposed to be in the list.
With lots of sex-related gore turning my stomach inside out, the Japanese flick included a huge penis that has sharp teech which really reminded me of Alien's tougue (there was also a similar scene with chest buster scene).:D

It's almost unfair to list some of the newer CGI stuff, so I'll keep my list organic:
Braindead is the king.
Premutos is an awful awful movie, but there's quite a bit of organic gore.

from what i understand it was done intentionally so that while the amount of gore onscreen was completely over the top it never actually looks real
House by the Cemetery was pretty damn gory, i could never get past the oh so very fucking annoying kid though, all i could think about during that film was how enjoyable it would be to strangle it

I probably don't have enough cred in this forum to cause a stink, but I would say that Salo is more shitty, that it is gory. And I mean 'shitty' in a very literal sense. Gross.
My last Netflix rental was Splatter: Naked Blood. It was bit slow, but by the end, there was plenty of gore.
I came across this somewhere (maybe here?). http://www.slaughteredvomitdolls.com/
It's the start of a new genre called 'vomit gore'. Looks a bit gimmicky to me, though.

Andreas Schnaas is a fairly terrible director, but the gore effects in his Violent Shit films are very well done and often pretty revolting. All of his movies are terrible, but the Violent Shit series is occasionally really effective, if only because of the quite convincing special effects work. The fact that the gore effects are above average, but the films were shot on a home video camera, adds to the effect. He even remade Anthropophagus, which has to be terrible. Regardless, the "testicle-piercing-with-fishhooks" scene in Violent Shit Part II is easily one of the most hard to watch moments in a film I've seen in some time.

I worked in the office on a vampire movie called The Thirst, and the dailies and what I saw of the rough cut were up there. All 'in-camera' effects too, everything done by hand. Every time I visited the set there were all these extras walking around covered in fake blood. Awesome. I think the director went on to re-make Wizard of Gore, so he's obviously got a knack for it!

I've been reading about this move recently called Philosophy Of A Knife, from the director of Nails and Visions of Suffering. Based on Unit 731 (same forgotten piece of history as Men Behind The Sun), 4 hours of documentary footage and images inter cut with a story following a nurse and a soldier in the unit... scenes of violence, torture and murder. The few reviews and comments I've come across from people online who have seen it claim it's one of, if not the most violent/gory movies ever made.

As far as best gore fest...well it depends- Audition isn't the goriest movie I've seen, but it's probably the most brutal. Cannibal Holocaust is probably up there in terms of disturbing/realistic gore, and Dead Alive is up there in comical over the top gore.

From http://blogs.ign.com/Horror_Brain/2006/06/29/23243/
The 10 Sickest Movies Ever Made
10.) Irreversible
09.) Men Behind the Sun
08.) Salo: 120 Days of Sodomy
07.) Ichi the Killer
06.) Murder-Set-Pieces
05.) Guinea Pig: Flower of Flesh and Blood
04.) Cannibal Holocaust
03.) Cutting Moments
02.) Aftermath
01.) August Undergrounds Mordum
and the honorable mentions- Scrapbook, DeadAlive, Visitor Q, Hellraiser 2, Mermaid in a Manhole, Nekromantik 1&2, Kichiku, Cabin Fever, Audition, Land of the Dead, Pink Flamingos
Not sure I agree with the list 100 percent but it seems about right. It is kind of weird seeing Hellraiser 2 and Land of the Dead in the honorable mentions and I had never heard of Cutting Moments or Aftermath until I read this list.
Another list I read, a top 20, was from someone who obviously didn't know what they were talking about - I mean the thing had Texas Chainsaw Massacre 2 and Resident Evil on it.

SPOILERS
I'm pretty sure the neck impalement was the babysitter, not the realtor. The realtor had a semi wince-worthy scene where her foot got stuck in the floor and she ripped it open trying to get away from the doctor.
And I didn't think Bob's parents scene was that gory...I don't even remember seeing Bob's mother get anything. I thought she was just dragged down the stairs, and that was the last anybody saw of her (she may or may not have hit her head on the way down, I can't remember).
END SPOILERS
My contribution (I'm not really into gore flicks) is Tenebre. There are some juicy bits throughout, but the end was the cherry; I won't give anything away, but about a half an hour from the end, there was a scene where I winced and went "DAAAAAAAAAAAAAAAAAAAAAAAAMMN!" . Fucking GREAT effects.

Gotta disagree with most of that, Dead Alive is the goriest flick ever, no questions asked. Day of the Dead is far gorier than Land. Pink Flamingos is not gory. Hellraiser 2 is deseving of an honorable mention. A lot of those flicks arent gory, just disturbing (or in the case of Pink Flamingos, sleazy.), and f u c k Murder-Set-Pieces.

Toxic Avenger, Dead alive, re animator they may not be the all time goryest but as far as gore fests that are actually good funny and scary films these are at the top of my list. Almost everything I watch is super gory bloody and violent though so what others consider a gore fest I may think is standard.
The most recent totally fucked up gorefest iv seen is gutterballs watched it online yesterday, not the best movie but in terms of gore and over all political incorectness it takes the cake. I found it more brutal than outright gory. To be frank, aside from the shock value, that movie is shite. Really cheap, badly made, and laughable in some places (like with that 70s disco-type score).
Reading all the responses around here makes me realize I've seen very little gore films... not that I'm a huge fan.
But I always thought Day of the Dead was pretty damn gory (and the effects are pretty realistic). There's a moment at the end where some poor bastard gets his head ripped off while he's screaming, and the scream gets horribly distorted as his voice box is ripped apart. That was incredibly painful to watch.
Or was that Dawn of the Dead?
And then there's Martyrs... not really full-on gory but it is fucking BRUTAL.
