I love Itilian horror, but I will be the first to admit that they can be quite strange. I think the strangest is CAT IN THE BRAIN by Lucio Fulci. In which the director stars as HIMSELF making horror films and going mad as murders are committed around him, which he thinks he may have committed. I don't think any other director would play with the public perception of themselves as madly as Fulci did. So I reckon it's the maddest of the mad, does anyone think there are any madder?

...since you are talking about "mad" directors I will bring up Abel Ferrara & "Driller Killer"...this is not Italian, but the director stars as the the killer in this classic from the seventies...was one of the big ones during the time period of "The Texas Chainsaw Massacre" & "Last House On The Left"...
...I always thought the Italian film "TENEBRE" was pretty "mad" just like a lot of Dario Argento's stuff...also don't forget the film "Baron Blood"...

Yeah, "Cat in the Brain" is definately one of the weirdest, but I'm particularly fond of Bava's "Lisa and the Devil." I love the idea that Ole Nick would place himself as a butler of the damned, catering to their weaknessess. (And Telly Savalas is great as a sardonic, lollypop-sucking devil, believe it or not. Look out De Niro and Pacino.)

Fulci (Ful-chee for me as well) is quite a mad hatter. I've only seen two of his flicks but they were both way out there. Zombie! and The Beyond were shown at late night screenings around these parts last year.
Great gory stuff but the plots were rather silly. The zombie vs shark scene in Zombie! was interesting to say the least.

Eaten Alive/The Emerald Jungle was a pretty messed up italian film. I can handle quite a bit, but this just made me sick!

Yeah cannibal films can be disgusting cause of all the animal violence who knows what they were thinking. I think Umberto Lenzi did Eaten Alive but I haven't seen it He also did a couple others: Man from Deep River and Cannibal Ferox probably others but I don't feel like looking it up. I own the Cannibal Ferox DVD and during his commentary he doesn't feel sorry for the animals he killed
Another would be Joe D'Amato who also directed porn. And mixed the two genre's together for Trap Them and kill Them a cannibal porn film
