More than the americans films I think.
Here are a few examples :
M from Fritz Lang ( about a serial killer who killed the little girls, unfortunately based on true events )
Funny Games ( SPOILER * SPOILER the whole family is killed !!! )
Zombie ( the splinter in the eye scene )
Inferno ( the rats scene )
A Blade In The Dark ( the shower scene )
Anatomy ( the victims are operated alive !! )
Open Your Eyes ( where's the reality, truely disturbing and cruel )
Los Sin Nombre ( aka The Nameless, don't expect a happy - end !!! )
etc ...
The europeans, especially the italians are really sadistic and cruel however I like that a lot.
