Need help identifying a song from the video

Okay, I saw this music video once, and I remember liking the song, but I have no idea what the song is. Can anyone help me?

Here's what I remember was in the video:

- scenes with an eye moving around quickly

- several scenes showing phrases like, "explicit content", "parental advisory suggested", "warning!", and other such phrases

- not 100% sure on this, but I think, at the end, you see a kid watching the video, then getting up, touching the screen, and disappearing or something

- I'm pretty sure the band Korn did the song

Anyone know what the song is?

I am 90% sure the band Korn did this song, I just can't remember the title.
