Does the director go around and do the interviews and everything?

Well, there's always a DVD producer. I'm guessing he's got a lot to do with it.

It also depends on the studio. If the studio says I won't give you money for the DVD, then no extras go to the DVD.

And I'm sure the director has some say in the whole thing. I'm sure if Spielberg says "no commentary" there will be no commentary.

A lot of stars/directors have been stipulating in their contracts that they will not do a commentary unless payed a huge amount of money and so we don't get commentaries because the studios feel it's not worth paying that much. There is always a seperate production team for the DVD and you can sometimes find these credits out by reading the literature of searching the disc, these people don't create the bonus material but rather find it and buy it, such as the featurettes etc. Trailers are the easiest to come by since the studio owns them and will probably offer them to the production team for free.
