More recent one must be from "28 days later".
My favorite opening scene is from Flatliners.
I mean c'mon it's just so cool.
Opera music, all these gothic shots, and then that coll line: "Today's a good day to die"
And then there is Final Destination 2

I'd have to say Day of the dead. The opening dream sequence which has dozens of zombie arms bust through a cement wall and grab Sarah was just classic!

Scream- The horror trivia game, the phone voice was cool and they killed Drew Barrymore.
Final Destination 2- The car crash scene wow.
Halloween- Love the point of view JC used then the shot of a young Michael coverd in blood holding the knife, great stuff.
Ginger Snaps- The dead dog and its owner screaming bloody murder and the pictures of death.
JAWS- The skinny dipping girl being attacked by a shark.
A Nightmare On Elm Street- An eerie atmosphere and introduced us to Freddy.

It didn't "scare the hell out of me" like I had been warned, but it was still an uber freaky opening scene.

Other cool openers:

Nightmare on Elm Street 3: Dream Warriors

Ginger Snaps: The pictures of death was pretty disturbing and so was its score over the top of it. It set us up for things to come.

I would say:
Scream: starts off a little normal, then slowly goes straight to hell with the phone calls.
Jeepers Creepers 2: Just seeing the Creeper hanging on the scarecrow bar thing was awesome.
Jaws: Pure classic, i wasnt really freaked out but it was still a powerhouse beginning to a classic movie.
Willard (Remake): The elevator scene when the door opens and rats spill out and he is just standin there all pissed off
TCM Remake: When the old guy goes bring it and the other guy goes bring what and Leatherface just comes up with a chainsaw
TCM: The guy goes in the house and Leatherface just comes up and beats the shit out of him with the hammar

Ghosthouse: Oh yeah! The deaths in the begining are pretty gruesome(Father gets an axe to the head, and a mother getting a knife slowly pushed into her throat against a door, thus pinning her to it!), plus, that nursery rhyme just gets to you, so damn eerie! The inflating lightbulb wasn't bad either...
House by the cemetery: A great death scene, not to mention it has great atmosphere! After a girl gets a large knife down into her skull and through her mouth, we see her get dragged away into the dark, then the opening credits start with a beautiful shot of a painting of the house in the movie, accompanied by the films great music score.
Mutant/Night Shadows: A cop investigates a house, goes into the basement, and is picked up by a large mutated man, and we see his shadow only while he snarls and the cop screams. Pretty good start to a decent zombie movie.
The Ring: What starts off with a Scream like opening quickly turns into a slow moving, very creepy begining to a great horror film/remake.
Ghost Ship: Great gore is all I'll say for Ghost Ships opening!
Shit sorry
i thought ya meant like when the monster or somethin like that is first shown

I love Donnie Darkos opening sequence with him lying on the middle of the road and then he just laughs..and then that cool song.
Also cool is Ghostbusters when they go and see that library ghost that screams at them.
Dawn of the Dead- the first 10 minutes are pure chaos. Still havent seen an opening sequence with more chaos and anarchy in the air. A real feeling of dread.
28 days later - this one is just eerie. Aside from the scenes where they free the monkey from the lab wich are preatty intense, the scenes with Jim walking around a desolated london are an incredible way to open a film.
Vampire Circus - The first 15 minutes of this film feel like you just walked into the last climatic scenes of some other movie. Count Mitterhouse shows no mercy upon the poor villagers who intend to kill him. We get a bunch of deaths, a little girl being killed, full frontal female nudity, and a vampire being staked in the heart in just the first few minutes of the film. What a grabber!
Nightmare on Elmstreet I - With the girl floating up in the air as Freddy kills her as her blood falls all over the room! That scene was a great way to open a movie.

Suspiria - It can't be beat in my opinion. From the moment Suzy steps out of the sliding doors and is engulfed by the rain and the pounding music, you know this is something special. The drive to the Dance Academy is mesmerizing with all its colors and rain, and that pan through the trees with the lights of the car flitting between them is incredibly eery.
Mute Witness - I love the fake B-movie that plays before the opening credits.
The Stendhal Syndrome - No dialogue at all in the opening scene. Masterful.
The Hitcher - Just love the photography, the rain, the scary hitcher... I'm counting everything until the infamous "FUCK YOU BUDDY!" line as the opening scene.
