I think when the mother eats her own ear in Braindead (Dead-Alive) is the worst thing i've seen recently.

By far the most shocking movie ever is Salo, but that doesn't really fit under horror.

Recent times for me, was the scene in Cannibal Ferox where the Mongoose gets slowly strangled to death by the Anaconda. Knowing that its life was really escaping before my eyes left quite an impact. Runner up for that film was the turtle, but really nothing sticks out as much as that mongoose. Simply horrifying.

Also noteworthy was when I rented Faces of Death at the age of 14. It wasn't so much scary, just so much that its horrifying element couldn't escape my mind for over a week. Then I found out later that most of it was fake anyways... eesh. I still remember drinking soda during the film, and at about midway I took a sip and what was once sweet tasted completely revolting. The power of shock video is unrelenting.
