Just watched The Suicide Club yesterday, there was some pretty twisted stuff in there.....still not sure how I feel about the film yet....

I would agree with most here about Cannibal Holocaust. I've seen the movie only once, about 6 months ago and there are images and sequences in that film that I couldn't get out of my head for weeks. It is truly a disturbing and sadistic film. I'm not as diverse as most on these boards in the horror genre so it might seem like a generic choice, but if you haven't seen the movie, give it a view.

More of a neo-noir psychological thriller than outright horror, but THE KILLER INSIDE ME with Casey Affleck, Jessica Alba & Kate Hudson is one mean-spirited little bastard of a movie! My second favorite film of 2010 right under SHUTTER ISLAND!

I watched it again recently...and psychologically (since there was minimal gore) it was very sadistic & cruel! The rape scene nightmare always makes my skin crawl! Mia Farrow deserved an Oscar for this film, but her real life kinda got in the way of reel life: her nasty divorce from Sinatra...what a fucking odd couple!

There's alot of sadistic films out there. And many have already been mentioned.

Umm... INSIDE, Frontier(s), Human Guinea pig 2: A Flower of Flesh and Blood, Salo: 120 Days of Sodom (the book is much more sadistic, however) |

The remake of 'I Spit on your Grave' is pretty intense. Very unsettling at parts.
