Just watched The Suicide Club yesterday, there was some pretty twisted stuff in there.....still not sure how I feel about the film yet....

I would agree with most here about Cannibal Holocaust. I've seen the movie only once, about 6 months ago and there are images and sequences in that film that I couldn't get out of my head for weeks. It is truly a disturbing and sadistic film. I'm not as diverse as most on these boards in the horror genre so it might seem like a generic choice, but if you haven't seen the movie, give it a view.

More of a neo-noir psychological thriller than outright horror, but THE KILLER INSIDE ME with Casey Affleck, Jessica Alba & Kate Hudson is one mean-spirited little bastard of a movie! My second favorite film of 2010 right under SHUTTER ISLAND!

Mia Farrow deserved an Oscar for this film, but her real life kinda got in the way of reel life: her nasty divorce from Sinatra...what a fucking odd couple!

There's alot of sadistic films out there. And many have already been mentioned.
But one fairly recent movie comes to mind for me. Martyrs. That movie really messed me up big-time.

The beginning of the remake has it's unsettling moments, however after that it gets to really fulfilling vindication that is a ton of fun.

I'd like to add Hellraiser to the list, I don't want to say it began the whole sadistic thing, but it really gets into the relationship between pleasure and pain and basks in it.
